Adirondack Rural Health Network 

The Adirondack Rural Health Network is a community partnership of 16 public, private and non-profit organizations in Upstate New York. Adirondack Rural Health Network creates a collaborative process for developing strategies and for implementing, monitoring and evaluating the regional health care system.

 

Adirondack Tobacco Free Network

The Adirondack Tobacco Free Network is a grassroots coalition committed to reducing adolescent and adult use of tobacco through cooperative programs in prevention, cessation, advocacy, policy initiatives and community education. The Adirondack Tobacco Free Network (ATFN) consists of local organizations and individuals from Clinton, Essex and Franklin counties. Our purpose is to promote a tobacco-free environment.

 

Association for Community Health Improvement 

The Association for Community Health Improvement is the premier national association for community health, healthy communities, and community benefit.

We convene and support leaders from the health care, public health, community and philanthropic sectors to help achieve shared community health goals, and work with hundreds of members to strengthen community health through education, peer networking, and the dissemination of practical tools.

Eat Well Play Hard North Country Coalition

The EWPH goal is to prevent childhood overweight and reduce long-term chronic disease risks through policy and environmental changes that will facilitate the promotion of physical activity and targeted dietary practices with children over the age of two.

Seven County Diabetes Network

The Seven County Diabetes Network is a coalition that is grant-funded through The New York State Department of Health. The Network targets individuals at risk for, or already diagnosed with, diabetes throughout Warren, Washington, Saratoga, Essex, Hamilton, Franklin and Clinton Counties. The Seven County Diabetes Network works collaboratively with healthcare facilities, public health departments and community organizations to increase public awareness of risks, prevention strategies, management issues and available local resources for people with diabetes or at risk of developing diabetes.
